Act of June 27, 1890

DECLARATION FOR INVALID PENSION.

State of Missouri , County of Wright , SS:

            On this 6th day of Sept, A. D. one thousand nine hundred and four (1904), personally appeared before me Sarah A. Chase a Notary Public within and for the County and State aforesaid Thomas H. Bittick, aged 64 years, a resident of the County of Douglas State of Missouri who, being duly sworn according to law, declares that he is the identical T H Bittick who was enrolled on the 31st day of Aug, 1862, in private Co L  28th Reg; and Thomas H Bittick, pvt Co I  28 Reg. Apr 30th 1864, Enrolled Mo. Mil, in the service of the United States in the war of rebellion, and served at least ninety days, and was honorable discharged Releived (sic) from duty Oct 26th 1862 and at Linn Mo., on the 27th day of Sept, 1864 and Relieved from duty Nov 17th 1864. 

That he has never been employed in the military or naval service otherwise than as stated above. 

That he is partially unable to earn a support by manual labor by reason of Rheumatism in Knees and Shoulders; heart trouble; partial deafness of the right ear; Catahr of the head; Kidney and bladder trouble and general debility  That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent.  That he has never before applied for pension under application No. _________  That he is a pensioner under Certificate No.-----  Also application for pension under the age limit under the ruling of the Honorable Pensioner Commissioner

That he makes this declaration for the purpose of being placed on the pension-roll of the United States , under the provisions of the act of June 27, 1890 .

He hereby appoints with full power of substitution and revocation, Joel F. Short of Mountain Grove Mo his true and lawful attorney to prosecute this claim, the fee to be Ten Dollars, as prescribed by law.  That his post-office address is Topaz, County of Douglas , State of Missouri

         Served also as private, Co I, 28 En Mo Mil from April 30, 1864 to Nov 17, 1864 .
